1. Full name(s) of the parent(s) or parent(s) to be.
2. Estimated Due Date or DOB of the baby. This is so we can put the parents on the wait list, if there is one, and so we know what size of kit they might need.
You will receive a PDF gift certificate within 24 hours which you can send on or print. We don't provide a physical certificate.
Terms:
Gift certificates are valid for 24 months from purchase.
Gift certificates are made out for 1 person/couple to redeem. They cannot be transferred to be used by another person/couple.
Gift certificates can not be sold on.
Gift certificates can not be refunded.
Gift certificates can only be used once.
Gift certificates do not guarantee that a rental trial kit will be available for the desired dates.
